| | There is no charge for rides. Donations to cover costs are appreciated. The Foundation is reimbursed for clients on Medicaid for trips to medical appointments. Within Deposit the suggested donation is $2.00. Trips to Sidney, Bainbridge, and Hancock have a suggested donation of $5.00. Trips to Binghamton and Endicott come with a suggested donation of $10.00. The suggested donation for a trip to Syracuse is $20.00. |

| | The Deposit Foundation provides volunteer-based, non-emergency transportation to clients living in any of Deposit’s three municipalities. Clients must have no or limited transportation. Transportation is provided to medical and other important appointments. The Foundation also schedules shopping trips on Saturdays alternating between the Vestal Wal-Mart and Sidney’s Price Chopper, K-Mart, and Frog Pond. |

| | The Deposit Foundation requires children under six years old to be secured according to the New York State Child Passenger Safety standards. |

| | Individuals with limited or no transportation who live in one of the three municipalities of Deposit (Town of Deposit, Village of Deposit, and the Town of Sanford) are eligible for transportation from the Deposit Foundation. |
| | A Rural Health Network associate rode along on a trip from Deposit to Lourdes Hospital. The Deposit Foundation has a 2005 Dodge Caravan minvan. The van was clean and air-conditioned. There were sliding doors on both sides of the van. A small wooden step was available for use in stepping into and out of the van. |

| | The driver pulled into the client’s driveway and rang the doorbell. Our passenger was waiting and ready to go when we pulled up. The driver assisted the client into the van and half an hour later, we arrived at Lourdes. We were ahead of schedule; the Deposit Foundation’s policy is to schedule an hour for the trip from Deposit to Binghamton, though the trip typically is shorter. The Deposit Foundation drivers are very accommodating as they make sure the client gets into the appointment location safely and on time. On one occasion, the driver recalled that she drove a client into Binghamton for a short-term stay at the hospital. The client realized that she forgot a few things at home. Rather than let the client stay in the hospital without them, the driver went to the local Kmart and bought the necessary items. |

| | The client’s appointment at Lourdes was over at its scheduled time. The driver returned the client to her house in the hills of Deposit. The doctor scheduled another appointment with the client who will contact the Deposit Foundation office to schedule the ride. |

| | The Deposit Foundation supports individuals living in the three municipalities in Deposit. Common destinations include medical facilities in Deposit, Windsor, Johnson City, Vestal, Binghamton, Endicott, Endwell, Walton, Delhi, Sidney and Syracuse. |
| | Service Hours: The van operates everyday except for Sunday. Hours are flexible. |

| | There is no service on New Year’s Day, Martin Luther King Day, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans’s Day, Thanksgiving, the day after Thanksgiving, Christmas Day, and the day after Christmas. |

| | The Deposit Foundation’s van has a step stool available. Drivers can help with walkers and canes. No wheelchair lift is available. Service is door to door so clients can count on help getting from the van to their home or destination. Assistance carrying groceries is also available. |

| | No animals are allowed in the Deposit Foundation van. |
| | Oxygen tanks are allowed. Individuals are responsible for their own oxygen tanks. |
